Your First Visit
Please arrive about 10 minutes early so that you have enough time to complete any necessary forms. You can fill out a New Patient Form at your first appointment, or download, complete, and bring it with you.
Your dentist will begin by reviewing your medical and dental history and completing a thorough dental exam. Your exam will include an examination of your gums, teeth, bite and jaw, an oral cancer screening and any x-rays that may be needed.
Based on your history and examination, your dentist will create a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

- How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. Please notify us of any change in plans as soon as you can, so another patient may be able to use the space in our schedule.
- Do you offer appointment reminders?
In the days prior to your appointment, you will receive automated phone appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
- Does your clinic have on-site parking?
Yes, free on-site parking is available for all our patients. You can also easily reach us via public transit.
Insurance & Financing
At Souris Family Dental, we'll help you navigate our options for payment and submission of insurance claims so you can focus on getting the dental care you need.
- Submission of Insurance Claims on Patient's Behalf
For your convenience, we can submit insurance claims on your behalf, resulting in faster reimbursement, without the need for paper forms and signatures.
Our administrative team can assist you in understanding your insurance plan better and can also send estimates to your insurance company prior to starting any major treatments.
Please bring your insurance plan information to your appointment.
- Payment Methods
You will be responsible for any co-pays or amounts not covered by insurance. These must be paid at the time of your appointment.
We accept payment by cash, cheque, debit or most major credit cards.
