Your First Visit*

Please arrive about 10 minutes early so that you have enough time to complete any necessary forms. You can fill out a New Patient Form at your first appointment, or download, complete, and bring it with you.

*IMPORTANT: Any form submitted by email must be encrypted, to ensure it is secure. By submitting an unencrypted email form, you are consenting to an unsecured email. Alternatively, please print off the form, complete it, and bring it to our office.

Initial Examination

Your dentist will begin by reviewing your medical and dental history and completing a thorough dental exam. Your exam will include an examination of your gums, teeth, bite and jaw, an oral cancer screening and any x-rays that may be needed.

Treatment Plan

Based on your history and examination, your dentist will create a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

• How can I book an appointment?

  Please contact our office during our regular business hours to book an appointment at Souris Family Dental.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please notify us of any change in plans as soon as you can, so another patient may be able to use the space in our schedule.

• Do you offer appointment reminders?

  In the days prior to your appointment, you will receive automated phone appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.

• Does your clinic have on-site parking?

  Yes, free on-site parking is available for all our patients. You can also easily reach us via public transit.

Insurance & Financing

At Souris Family Dental, we'll help you navigate your options for payment and submission of insurance claims so you can focus on getting the dental care you need.

• Submission of Insurance Claims on Patient's Behalf

  For your convenience, we can submit insurance claims on your behalf, resulting in faster reimbursement.

  Our administrative team can send estimates to your insurance company prior to starting any major. 

  Please bring your insurance plan information to your appointment.

• Payment Methods

  You will be responsible for paying for treatment in-full at the time of your appointment. We are happy to work with you to help you submit dental insurance claims.

  We accept payment by debit or most major credit cards.
